Organisation Change Management is the Process of changing an Enterprise.

Why
Enterprises can expect an overall productivity decrease during the Rollout of the new Application System. This can be shown diagrammatically as follows:

Having staff work longer hours to keep both the legacy and the new Application Systems running

The learning curve (and frustrations) associated with the new Application System

Potential problems with the introduction of the new Application System (eg, “bugs”)

Potential problems and learning curve associated with staff using changed work practices
One of the major responsibilities of the Project Co-Ordinator will be to minimise both the duration and the severity of the productivity decrease through project planning and the development of Risk Avoidance and Mitigation strategies.
The introduction of a new Application System will see many existing manual Procedures being automatically performed by the new Application System. Most of these manual Procedures will be low-value, menial activities like duplicate data entry, reconciling results from two different systems, filling in forms, manually walking or telephoning work requests from one person to another, etc. This can best be seen from the following diagram:
Not only will the existing manual procedures be subsumed, they will also be standardised by the new Application System. Most Enterprises find that the existing manual procedures are being performed in a number of different ways, typically caused by geographical remoteness, parochial or historical interests.

This technique is used throughout the whole project lifecycle and is not limited to any specific project lifecycle phase.

Organisation Change Management = implementing the vision and strategy on a day to day basis (ie, tactics).
Needs to be managed by Project Co-ordinator and/or Programme Director and/or Sponsor with assistance from the Organisation Change Manager.
Even if you do not have Business Process Re-Engineering (BPR) in your project scope, there will still be many changes in the way people perform their jobs (and not just on the computer).
Effective communications are essential.
Managing Expectations is essential.
Process improvement is, and must be, a grassroots effort for it to succeed.
